    Assembly and analysis of fragmentation data for liquid propellant vessels

    Fragmentation data was assembled and analyzed for exploding liquid propellant vessels. These data were to be retrieved from reports of tests and accidents, including measurements or estimates of blast yield, etc. A significant amount of data was retrieved from a series of tests conducted for measurement of blast and fireball effects of liquid propellant explosions (Project PYRO), a few well-documented accident reports, and a series of tests to determine auto-ignition properties of mixing liquid propellants. The data were reduced and fitted to various statistical functions. Comparisons were made with methods of prediction for blast yield, initial fragment velocities, and fragment range. Reasonably good correlation was achieved. Methods presented in the report allow prediction of fragment patterns, given type and quantity of propellant, type of accident, and time of propellant mixing

    REASSURE: A Self-contained Mechanism for Healing Software Using Rescue Points

    Software errors are frequently responsible for the limited availability of Internet Services, loss of data, and many security compromises. Self-healing using rescue points (RPs) is a mechanism that can be used to recover software from unforeseen errors until a more permanent remedy, like a patch or update, is available. We present REASSURE, a self-contained mechanism for recovering from such errors using RPs. Essentially, RPs are existing code locations that handle certain anticipated errors in the target application, usually by returning an error code. REASSURE enables the use of these locations to also handle unexpected faults. This is achieved by rolling back execution to a RP when a fault occurs, returning a valid error code, and enabling the application to gracefully handle the unexpected error itself. REASSURE can be applied on already running applications, while disabling and removing it is equally facile. We tested REASSURE with various applications, including the MySQL and Apache servers, and show that it allows them to successfully recover from errors, while incurring moderate overhead between 1% and 115%. We also show that even under very adverse conditions, like their continuous bombardment with errors, REASSURE protected applications remain operational

    The cost-effectiveness of expanding the UK newborn bloodspot screening programme to include five additional inborn errors of metabolism

    Glutaric aciduria type 1, homocystinuria, isovaleric acidaemia, long-chain hydroxyacyl CoA dehydrogenase deficiency and maple syrup urine disease are all inborn errors of metabolism that can be detected through newborn bloodspot screening. This evaluation was undertaken in 2013 to provide evidence to the UK National Screening Committee for the cost-effectiveness of including these five conditions in the UK Newborn Bloodspot Screening Programme. A decision-tree model with lifetable estimates of outcomes was built with the model structure and parameterisation informed by a systematic review and expert clinical judgment. A National Health Service/Personal Social Services perspective was used, and lifetime costs and quality-adjusted life years (QALYs) were discounted at 1.5%. Uncertainty in the results was explored using expected value of perfect information analysis methods together with a sensitivity analysis using the screened incidence rate in the UK from 2014 to 2018. The model estimates that screening for all the conditions is more effective and cost saving when compared to not screening for each of the conditions, and the results were robust to the updated incidence rates.     An Experimental Evaluation of Deliberate Unsoundness in a Static Program Analyzer

    Abstract. Many practical static analyzers are not completely sound by design. Their designers trade soundness in order to increase automa-tion, improve performance, and reduce the number of false positives or the annotation overhead. However, the impact of such design decisions on the effectiveness of an analyzer is not well understood. In this pa-per, we report on the first systematic effort to document and evaluate the sources of unsoundness in a static analyzer. We present a code in-strumentation that reflects the sources of deliberate unsoundness in the.NET static analyzer Clousot. We have instrumented code from several open source projects to evaluate how often concrete executions violate Clousot’s unsound assumptions. In our experiments, this was the case in 8–29 % of all analyzed methods. Our approach and findings can guide users of static analyzers in using them fruitfully, and designers in finding good trade-offs.

    Projected effects of disruptions to human immunodeficiency virus (HIV) prevention services during the coronavirus disease 2019 pandemic among Black/African American men who have sex with men in an Ending the HIV Epidemic priority jurisdiction

    BACKGROUND: Disruptions in access to in-person human immunodeficiency virus (HIV) preventive care during the coronavirus disease 2019 (COVID-19) pandemic may have a negative impact on our progress towards the Ending the HIV Epidemic goals in the United States. METHODS: We used an agent-based model to simulate HIV transmission among Black/African American men who have sex with men in Mississippi over 5 years to estimate how different reductions in access affected the number of undiagnosed HIV cases, new pre-exposure prophylaxis (PrEP) starts, and HIV incidence. RESULTS: We found that each additional 25% decrease in HIV testing and PrEP initiation was associated with decrease of 20% in the number of cases diagnosed and 23% in the number of new PrEP starts, leading to a 15% increase in HIV incidence from 2020 to 2022. CONCLUSIONS: Unmet need for HIV testing and PrEP prescriptions during the COVID-19 pandemic may temporarily increase HIV incidence in the years immediately after the disruption period

    Diagnostic accuracy of biopsy versus full excision for diagnosis of oral cancer: a systematic review

    Objectives: To review studies assessing diagnostic accuracy of biopsy of oral lesions for diagnosis of oral cancer and potentially malignant disorders, compared with full excision. Methods: Systematic review: four databases were searched for studies conducted within developed countries and published between 2009 and January 2020. Results: Six studies met inclusion criteria, with wide variation in methods and results. For identifying dysplasia or malignancy, sensitivity of biopsy (versus excision) ranged from 42% to 86% and specificity from 75% to 100% across two studies, while concordance varied from 27% to 89% across four studies. For identifying malignant-only lesions, sensitivity was 71% and 94% in two studies, while specificity ranged from 17% to 100% across four studies. Conclusions: There are few published studies assessing biopsy accuracy, with varying results. Further research should evaluate factors impacting accuracy, such as biopsy depth; multiple biopsies of large lesions; discordance between pathologists; and regular follow-up

    Systematic review of newborn screening programmes for spinal muscular atrophy

    Spinal muscular atrophy (SMA) is a genetic neuromuscular disorder causing the degeneration of motor neurons in the spinal cord. Recent studies suggest greater effectiveness of treatment in the presymptomatic stage. This systematic review synthesises findings from 37 studies (and 3 overviews) of newborn screening for SMA published up to November 2023 across 17 countries to understand the methodologies used; test accuracy performance; and timing, logistics and feasibility of screening. All studies screened for the homozygous deletion of SMN1 exon 7. Most (28 studies) used RT-PCR as the initial test on dried blood spots (DBSs), while nine studies also reported second-tier tests on DBSs for screen-positive cases. Babies testing positive on DBSs were referred for confirmatory testing via a range of methods. Observed SMA birth prevalence ranged from 1 in 4000 to 1 in 20,000. Most studies reported no false-negative or false-positive cases (therefore had a sensitivity and specificity of 100%). Five studies reported either one or two false-negative cases each (total of six cases; three compound heterozygotes and three due to system errors), although some false-negatives may have been missed due to lack of follow-up of negative results. Eleven studies reported false-positive cases, some being heterozygous carriers or potentially related to heparin use. Time to testing and treatment varied between studies. In conclusion, several countries have implemented newborn screening for SMA in the last 5 years using a variety of methods. Implementation considerations include processes for timely initial and confirmatory testing, partnerships between screening and neuromuscular centres, and timely treatment initiation

    Diagnostic accuracy of conventional oral examination for detecting oral cavity cancer and potentially malignant disorders in patients with clinically evident oral lesions: Systematic review and meta‐analysis

    This systematic review evaluates the diagnostic accuracy of conventional oral examination (COE) versus incisional or excisional biopsy for the diagnosis of malignant and/or dysplastic lesions in patients with clinically evident lesions. Searches were conducted across five electronic databases from inception to January 2020. Meta-analyses were undertaken, where appropriate. Among 18 included studies, 14 studies were included in the meta-analysis, giving summary estimates for COE of 71% sensitivity and 85% specificity for the diagnosis of dysplastic and/or malignant lesions. The pooled diagnostic accuracy of identifying malignant-only lesions was reported in seven studies, giving a pooled estimate of 88% sensitivity and 81% specificity. Diagnostic accuracy of different types of dental/medical professionals in identifying dysplastic or malignant lesions gave varying estimates of sensitivity and specificity across three studies. Further research is needed to improve the diagnostic accuracy of COE for early detection of dysplastic and malignant oral lesions
